Consumer Health Data Privacy Policy

This policy applies to consumer health data handled by Shop Holistico Inc. (“Holistico,” “we,” “us”) as defined by Washington’s My Health My Data Act, Nevada’s consumer health data law, and similar laws.

When you buy the Cell Science Systems Cellular Micronutrient Assay through our link, we collect your name and the fact of your purchase. As the referring practitioner, we also have access to your test results, which include your date of birth, in DHA Laboratory’s practitioner portal. If you ask us to review your results with you, we may keep notes from that review.

We use your name and purchase information to verify and reconcile our commissions. We view your test results only if you ask us to review them with you. During a review, we may recommend products, including Holistico products, which we make and sell.

We receive this data from DHA Laboratory, which sells and processes the test, or from you.

We share this data only with service providers that securely store our business records and with professional advisers such as our accountant. We have no affiliates that receive it. We do not sell consumer health data, and we do not share it with advertising partners.

You have the right to:

• confirm whether we collect, share, or sell your consumer health data
• access it
• get a list of all third parties and affiliates we shared it with, and their contact information
• delete it
• withdraw your consent

Your test results are stored by DHA Laboratory, not by us. To have them deleted, or to remove our practitioner access, please contact DHA Laboratory. We will help you with that request if you let us know.

To exercise these rights, email hello@shopholistico.com with the subject line “Consumer Health Data Request.” We will respond within 45 days, extendable once by 45 days when reasonably necessary. If we deny your request, you can appeal by replying with the subject line “Consumer Health Data Appeal.” If your appeal is denied, you may contact the Washington State Attorney General at atg.wa.gov.
